First begin by properly tensioning your chain.  A loose chain will cause the chain to fall off:

1.     Loosen the wheel axle nuts

2.     Pull the wheel backwards, placing tension on the chain

3.     While forcing the wheel backwards, retighten the axle nuts

 

Next, ensure that the wheel is aligned straight, parallel to the frame.  A misaligned wheel will cause the chain to fall off.

 

Then, inspect the chain for bent links.  Also remove the front chain guard and inspect the front chain wheel to ensure it is straight and not bent.  


 

In rare cases, try loosening this screw until the chain remains on the drive train while turning the cranks. If you wish to tighten the screw firmly then you must have additional washers between the chain guard and the frame.  See picture.
